The HCPTS panel approved occupational therapist Rebekah Gibson's immediate voluntary removal from the register. She admitted serious failures in risk assessment, discharge planning, equipment, record keeping and safeguarding involving two service users. Because she had left the profession and did not intend to return, the panel found removal equivalent to striking off adequately protected the public.
