The review panel received no information showing remediation or an intention to address the shortcomings, and decided another suspension would achieve nothing. Charges
The allegation concerned failures between May 2019 and February 2020 to keep full clinical records, document interventions and review service users on time. It also alleged that the Registrant recorded seeing a service user after that person's death and completed another service user's notes more than five months after a home visit. Particulars 4, 5 and 6 were not proved. The alleged matters were said to amount to misconduct and/or lack of competence and impairment.
Findings
The final hearing found record-keeping and review failures involving numerous service users, the post-death entry and the delayed notes proved, while other factual elements were not proved. It found serious misconduct and imposed a 12-month suspension. At review, the current Panel found the Registrant remained impaired because he provided no evidence of remediation or intention to return to practice. It replaced suspension with striking off from 5 September 2025.
